In Times of Uncertainty; Clinical Practice

H. Colodro and J. Oliver provide sound advice in their new book, A Guide to Self-Care for Practitioners in Times of Uncertainty. This 2020 New Harbinger publication is loaded with helpful suggestions on surviving, even thriving, in our time of struggle. Their core questions include: What do my clients need most now? Am I able to provide what is needed and do so effectively? What might I do regarding my own uncertainty and struggles? Here are some of their suggestions.

  1. Spend some time daily practicing self-compassion.
  2. Know what you are facing, and if you are in a position (emotionally) to provide what is needed.
  3. Recognize the effects of chronic stress on yourself and your clients. Make adjustments as needed.
  4. Work smart! Work with only what you have some control over and not what is beyond your control.
  5. Be sure to take good care of yourself: sleeping, eating, attending, worrying, being aware, obtaining social-emotional support, meditating/doing yoga, and holding onto a strong sense of consciousness.
  6. Check in with yourself. Ask: How am I doing right now? Why did I react emotionally to that? etc.
  7. Release yourself from a “fix-it” mentality. Rather focus on emotion regulation, stability, and safety.
  8. Be there with your own, personal losses, sadness and fear. Deal with these and seek help when needed.
  9. If appropriate, include happiness-enhancing activities and skills in your therapy.
  10. Recognize the reality of impermanence in all things, including current problems and yourself.
  11. Settle into a more self-caring flexible outlook. If possible, work less. Take breaks! Do not over-extend.
  12. If you experience psychological exhaustion or failure in behavioral activation, seek support and consider doing more telehealth rather than live, person-to-person therapy. Cut back if necessary.
  13. Follow all guidelines regarding masks, gloves, social distancing, disinfecting, etc.
  14. Consider using “good enough” standards in your work in these times. See D.W. Winnicott for details.
  15. Remain safe, calm, competent, and helpful in your work.
  16. Consider using the same breathing training techniques you teach to your clients on yourself.
  17. When you feel stuck or sucked-into the craziness of the times, use CBT-M (CBT with mindfulness) on yourself.
  18. If you feel overwhelmed, hopeless, etc. seek your own clinical help.

A Tribute to Ram Dass (Richard Alpert)

Recently Ram Dass died at his home in Maui. He was 88 years old.  He was born into a well-off Boston family, and enjoyed materialism in his early professional years.  When completing a Ph.D. in Psychology at Stanford University he was still into material things. His spiritual awakening did not exist.  He was once known for his psychedelic drug experimentation while teaching at Harvard University. This experimentation occurred in the 1960s and 1970s, and in 1963 he was fired from Harvard for related reasons. In 1967 he went to India to study under Hindu Sadhu Neem, Karoli Baba, also known as Maharaj-ji. Before his teacher died, Alpert was named Ram Dass (Servant of God). He practiced bhakti yoga – pure love on the spiritual path. In 1974 Ram Dass returned to the United States, where he developed his own style of meditation, a style that radically integrated various forms of spiritual traditions.  In 1971 he published his first book, Be Here Now;  this book helped open up higher consciousness to others without the use of drug and expounded a life role of service to others. In 2004 Ram Dass relocated to Maui, where he remained for the rest of his life.  In 1997 he wrote Still Here. In some ways this book helped to clarify his deeper understanding of himself; the depth was the result of a serious stroke he suffered. His cerebral hemorrhage forced him to go deeper into self-understanding. In 2004 he survived a near-fatal infection. From his stroke on Ram Dass  was mainly homebound. His last book (with Mirabai Bush) was Walking Each Other Home:Conversations on Living and Dying. 

To the end he faced his death with great human grace, and he never gave up his view that to live must involve to love and serve others. Ram Dass, sometimes with the help of others, also established various organizations to provide teachings and service to others. Here is a short list: Hanuman Foundations (the teachings of Neen Karoli Baba); Love Serve Remember Foundation; Prison Ashram Project; Co-creating Living Dying Project and Doorway to Light; Seva Foundation (healthcare for underserved areas); and, Social Venture Network (for businesses). Yes, Ram Dass lived his talk – he walked the walk!

With the passing of Ram Dass, we have lost a great spiritual and meditation teacher. There are few and far between equivalent replacements. May he rest in great inner peace as he travels the bardo.

Crisis Resilience Skills  – Mindful Happiness

Below I will list various interventions that have proven effective in reducing the level of personal crisis. The sources for many of these skills came from Burns (1980), Ellis (1995), Seligman (1988), Linehan (1993, 2015)), Hayes (2018), and Thich Nhat Hanh (various publications). The skills noted are for immediate application in crisis and/or post-crisis practice. Due to space limitations, I will not explain details; rather I will list skills with minimum directions. If interested in improving your clinical capacities to deal with crises, you can look up the details on your own. It is a growth process. It is always a good idea to have a clear and practical crisis response plan.

  1. Move to cognition as soon as possible – get out of body reactions and take over the thought process related to the situation. Practice Tara Brach’s RAIN skills (recognize, accept, investigate, and relate to non-self), complete a pros and cons grid (good and not-so-good things about staying the same versus making small changes – CT, MI, CBT). Also distant or distract yourself quickly. Distraction is not to be used in physically dangerous situations.
  2. Practice mindfulness core skills. Begin relaxation breath with deep, slow breathing (polyvagal impediments may exist especially if poorly treated trauma is a reality), use positive imagery, meditate, do yoga, pray, pay attention to non-crisis variables, and live within the realities impermanence.
  3. Practice self-soothing. Remember or engage in positive images, sounds, touch, smells, and tastes. Carry your favorite self-calming scent with you. Rub your hands hard and long until hot, then place them on your face and absorb the healing warmth.
  4. DBT-like skills are highly effective. Use “wide-mind” skills. Try ACCEPTS. Engage in alternative activities, contribute to others, compare downwardly with others, engage in opposite emotion, push away unhelpful thoughts and move away from the situation, engage in productive thinking about what to do now without emotional dysregulation, and improve your sensations. Although not part of DBT, you may wish to practice progressing counting (distractive); say to yourself or outloud consecutive numbers and imagine them in your mind’s eye. Continue to count until the emotional reactivity has reduced.
  5. Practice mindful movement. Do yoga, tai chi, qi gong in more vigorous modes until you notice that your body has experienced a reduction in emotional reactivity. Regular meditation practice is, perhaps, your best option here.  Do vigorous exercise.
  6. Do your best to reduce a “victim” self-image. Work on fear-based reactions and combat hopelessness and helplessness tendencies. Use your older, experienced self’s wisdom.
  7. If in therapy, be certain to process the crisis experience. If your therapist is competent, she/he will include such skills development as part of your treatment.
  8. Hope this quick review has been helpful to you.

“i Rest” Yoga Nidra Practice (Richard Miller, Ph.D.)

All regular meditation and yoga practices are capable of bringing us closer to our true self and our relationships in the world. A by-product is deep relaxation and equanimity. Richard Miller, Ph.D. (Clinical Psychologist, yogic scholar, spiritual teacher), has created a yoga nidra practice that promises to return us to our authentic self and provide deep, relaxation and inner peace in the process. The iRest method is based on long-term practice and research. Yoga nidra is a pathway to improved health, awareness, and inner healing. It has been effective in problems related to stress, anxiety, sleep, trauma, and physical pain. I suggest it may be equally effective in all psychological pain. In neuroscience theory yoga nidra may reset your central nervous system, improve coping capacity, and open doorways to spiritual development.  In the following yoga nidra meditation I have integrated my own yoga nidra practices with the iRest method presented by Dr. Miller. Let’s begin!

  1. Place your body in a comfortable positions sitting, lying down, or standing. Enjoy silence in the pause. Close your eyes if ok.
  2. Allow your body to settle down and simply notice your full awareness. Notice silence in the pause.
  3. Open up your senses without evaluation, and notice delicately your body contact with parts of the world beyond the body boundary. Pause.
  4. Be open to noting the contact experience, and prepare to go inward inside your body. Notice as we pause in silence.
  5. Gently notice without evaluation the sensations you feel as we move up and down the body. Quickly at first. Up and then down!
  6. Starting with your toes, simply notice sensations; then move up slowly all the way to your hips, noticing sensations as you move your intentional kinesthetic awareness. Be with the silence in the pause.
  7. Continue your inner, private body scan moving from the hips all the way up your chest and to the top of your head. Silence!
  8. Just notice sensations and continue to be aware without judging.
  9. Now move  slowly from the head back down your back to the hips. Notice the quality of the silence in the pause.
  10. Again, with intentional attention without thinking move down from the hips all the way back to the toes. Remain silent inside.
  11. Now ever so gently notice your intentional in and out breaths. Breathe deeper, slower. Notice.
  12. Notice and allow the sensations of breath passing into your nostrils, down the throat, into the chest and lungs. Notice the rise of the belly on the in-breath.  Do all this without thinking – only noticing the sensations your feel. Enjoy this silent pause. Let go.
  13. Now notice the out-breath and follow the same sensation track as your exhale. Notice! Remain silent inside and outside.
  14. At this point in our process, bring attention gently to an area of the body where you feel discomfort, pain, or suffering.
  15. Simply allow and welcome this sensation as if it were simply sensations – like those you just experienced. Pause in silence.
  16. Blend the earlier pleasant sensations with this uncomfortable sensations. Notice!  Settle deeply into the silent pause.
  17. In your effort to accept all sensations, notice the discomfort and note if it is stronger at its center or at the periphery. Where?
  18. Focus more attention where there is less discomfort. Go deep into the silence here.
  19. Does your unpleasant sensation have a color? Focus on the color. Now focus on an opposite color or your favorite color. Notice.
  20. Let go of reactions to the discomfort on each and every exhalation, and especially in the gaps/spaces between breaths and awarenesses.  Be with the silence now.
  21. Continue to let go in the longer silence here.
  22. Does the sensation have a character texture?  Is it dense or loose? Heavy or light? Sharp or dull? Go to where there is less discomfort. Be in your inner most silence now.
  23. Radically accept all your bodily sensations – the pleasant along with the unpleasant. It is only sensation; it is your reaction to it that causes you problems. We wish to flee discomfort and hold onto pleasure. Just be with what is!  Just be in silence now.
  24. Allow your body to remain aware, accept, integrate all the forms of sensation you are experiencing.
  25. Examine your control by bringing focus to the pleasant sensations, now to the unpleasant sensations. Accept them all as simply sensations without evaluating. This may be challenging. Find some peace in the silence.
  26. Now allow your senses to open up and notice what you feel, hear, see, taste, smell. etc.
  27. Use the power of your inner spirit to help you improve this moment of perfect being. Empower spirit! Sit in silence now.
  28. When you feel ready slowly bring yourself to a full alert state. If your eyes have been closed, gently open them.
  29. Now just rest!!!!! Enjoy the silence. I will cue you when we are ready to end yoga nidra.

The Deep Courage to Let Go

Pema Chodron, now recognized as a world leader in the Chogyam Trungpa Shambhala tradition, has presented a wonderfully clear method for letting go of personal blockages and impediments to enlightenment,  the bodhisattva way of life, and awakened bodhichitta (clear mind, soft heart). She teaches us how in “The Joy of Letting Go.” It is similar to accepting “choiceness awareness” in one’s own life experiences. Chodron suggests that we radically accept whatever arises in our path in life as our opportunity to practice. Our job is to reduce the suffering of the world – one person at one time. This practice may be achieved through the Paramitas. The practice of being patient, disciplined, generous, energetic – all in meditation and life behaviors – sets the stage for our inner and outer growth. As we practice greater compassion, we care more deeply about the welfare of others. This can be an antidote to the pandemic of greed in America (and the world) today. So much greed that we may inflict great pain and suffering on others. How much wealth is enough?

One way to experience this process is to be highly generous as a personal life aspiration. By giving to others we will notice the inner emotional reactions in letting go of things we value. This implies letting go not only of the “things” but also of the attachment to the things we value. Our wholesome actions in giving allow us to experience first-hand the reality of holding on. Just how difficult is it for you to let go, give away that thing you value so much? Our improving discipline in practices tames the wild mind and expands open-hearted compassion for others, especially others who lack what we have. This awareness enhances personal gratitude for it all. We humans sometimes “own” many things; sometimes this property restricts own flexibility in life. It certainly adds fear of loss. It increases our level and intensity of grasping and competing. If you go hiking while carrying an armful of firewood, you will soon realize it is very inefficient. So to go through life with a household of “stuff” is a logical understanding of this hiking metaphor. It is in the giving away, the letting go, that true and personal liberation is experienced.

Taming our reactive minds and bodies is another important practice in letting go. Can we really be “like a log of wood” (influence of Shantideva and others) when emotionally powerful experiences occur? This does not imply suppression, repression, or being a doormat; it implies building better emotion regulation skills through practice. Slow it all down, clarify your discernment, and allow enthusiasm for making such positive personal changes. It is all up to you.
